Propranolol

What is the use of Propranolol and how it works?

It decreases heart rate, force contraction and cardiac output. On prolonged administration BP gradually falls in hypertensive but not in normotensives.

Indications of Propranolol

Hypertension, angina pectoris, cardiac arrhythmias, post M.I. tachycardia, migraine, anxiety.

Dosage of Propranolol

10 mg BD to 160 mg QID (average 40-160 mg/day)

Contracdictions of Propranolol

Obstructive (COPD) (asthmatics). Partial or complete heart block. CHF acute MI cardiogenic shock. Digitalis and verapamil therapy.

Precautions of Propranolol

Impaired renal and hepatic functions, pregnancy, cardiac failure. Myocardial insufficiency reduced cardiac reserve.

Drug Interactions of Propranolol

Hypertension & bradycardia with adrenaline. Reduced heart rate & output with anesthetic agents. Severe bradycardia may occur with digitalis and calcium channel blockers. Elevated blood levels with cimetidine. Hypotensive effect reduced by indomethacin. Additive effect with other antihypertensives and diuretics.

Side effects of Propranolol

Tiredness, lack of drive, GI upsets, nightmares, forgetfulness, rarely hallucinations, muscular fatigue.
